What does the day look like?
Between 80% and 90% of your working week is dedicated to spreadsheets, staff appraisals, departmental budgets, recruitment, and regulatory audit compliance. You manage clinical operations using electronic patient records like Epic Systems, Cerner Millennium, SystmOne, or EMIS Health, track incident investigations on Datix or Ulysses, and plan rosters and payroll via Allocate HealthRoster and ESR. You also write business cases for service expansion and negotiate service level agreements with school districts, integrated care boards, and stroke teams.
Direct patient contact accounts for only 10% to 20% of your time, reserved for the most complex clinical situations. This involves conducting FEES (Fiberoptic Endoscopic Evaluation of Swallowing) assessments, videofluoroscopy swallowing studies, and medicolegal reviews. The standard working week is 37.5 hours in the UK NHS or 40 hours in the US and Australia, running Monday to Friday from 08:30 to 16:30 or 09:00 to 17:00. There are no regular night shifts, though you may cover weekend escalation rotas for acute stroke or tracheostomy units.
